Skip 2a and 2b.


Enter your current address in space 3.


If you filed for 2008 or an earlier year, and your address on your last tax return was different from your current address, enter the address from your last tax return in space 4. If you have never filed or your address on your last tax return was the same as your current address, skip line 4.


Skip line 5 if you want the transcript sent to you. If you want it sent directly to the college financial aid office, enter that office's address in space 5.
